The OU is only aware of one infant formula that is certified Kosher for Passover and available in the States. The brand is called Materna. It is made in Israel and is under the Certification of the BaDatz, when their certification appears on the product. Their products, both dairy and parve, are available in many outlets in numerous Jewish communities. Their website is http://maternausa.com.
If your child needs formula you may use one of the products listed below. These products have been reviewed and can be assumed to be free of chametz. However, many of these products contain kitniyoth, sometimes as a primary ingredient. It is important though that you purchase these products at least a day before Pesach.
All the products on this list are acceptable in concentrated liquid form, ready-to-use liquid form, or powder form. In liquid products the kitniyoth in the product is butel b’rov, or nullified in the product.

Most infant formulas are made from soy products. At first glance, one might assume that this presents a problem of Kitniyoth on Passover.
However, the custom of disallowing the use of ‘Kitniyoth’ (soy, for example) was never applied to infants nor to people who are ill. However, you must take care to keep bottles, nipples and formula away from the general kitchen area. Any mixing or washing should be done in the bathroom sink and NOT in the kitchen sink.
The Passover problem with most Kosher baby formulas has to do with the soy or possible corn ingredient. [Any possible ascorbic acid, vitamins, or other possibly Chametz items added are Halachically nullified]
If we know that there are no ingredient from the five types of grain (wheat, barley, rye, oats or spelt) in the infant’s formula, then there is only a concern about the Rabbinic prohibition of eating legumes (such as beans, peas, lentils, rice, soya beans, maize, or corn, to give just a few examples of what is in the category of Kitniyoth) on Passover.
Despite the fact that Kitniyoth for infants is permissible, the Mishna Brura states that it is preferable (but not mandatory) to avoid giving infants rice, buckwheat (kasha in Yiddish), or millet on Passover.
Never the less, should there be circumstances that require that the infant eat one of the above types of Kitniyoth, they may certainly be given to them.
We have indicated that Kitniyoth is permitted for infants – but that does NOT cover Chametz, or any food that was processed on equipment that was not properly cleaned for Passover. Such Chametz is strictly prohibited even for infants.
When we talk about Kitniyoth being permitted for infants, it means that it would have to be a homemade Kitniyoth, unless the Sephardic community has a reliable Hechsher on an infant product.
The Halacha (Jewish law) also suggests that separate pots, dishes and cutlery be dedicated to the Kitniyoth food.
Obviously, in cases whether a doctor insists that the infant must have the Chametz, the prohibition does not apply. In such a situation, it would be preferable (if possible) that a gentile should bring his own Chametz and feed the baby. If that is not feasible then a Jew would be permitted to feed the infant Chametz.
The upper age still considered an ‘infant’ depends on the child. Obviously, if the child can eat healthy food that is not Kitniyoth, that would be preferable. Usually, the Poskim indicate that two years of age would be the upper limit, but if a 2-year old is not ready to eat other food, then certainly the limit goes up as high as his or her Bar or Bat Mitzvah.
